Prescriptions/SureScripts
Our practice is committed to the safety, security and accuracy of your prescription. For this reason we send most of our new prescriptions and refill requests electronically to your pharmacy using a secure network called SureScripts, an electronic prescribing network. Please indicate to you pharmacist to check for your prescriptions or refills by this method, as well as by fax. If you are using a mail-order pharmacy, please have the location of the pharmacy and the fax number you want used, along with a member ID number if it
is required. We do not routinely prescribe or refill antibiotics, narcotics, or sedatives over the phone. These types of medications need to be discussed with your provider.
For prescription refills, please call your pharmacy at least 48 hours in advance, Monday-Friday- EVEN IF YOUR PRESCRIPTION STATES: NO REFILLS. The pharmacy will call us for verification. Be sure to call for a refill before your medication runs out! Plan ahead for week-ends and holidays. Be advised that it may take 36 to 72 hours to process your refill request.

Medical Records
Your medical records are confidential and will not be released without your signed authorization. If you wish to have a relative, friend or caregiver able to access your medical records, please print and fill out the release of information page that allows access and bring it to your appointment. This will be saved in your information for future reference. We have recently implemented electronic medical records (EMR) which will provide our staff and providers quicker access to your information. You can also download release of information forms if you want to transfer records to or from us. These forms are located on our forms page.


Referrals
As a primary care practice, we can take care of most of your health care needs but for more complex issues we can refer you to a specialty provider. Most insurance companies require you to have a formal referral in place before you make your appointment with the specialist. Part of this process is to see your provider for an evaluation in order to make an informed decision about where and to whom to refer you. We will check your insurance panel and will inform you of the specialist you have been authorized to see. It is important to understand the guidelines that your insurance and specific plan requires, so please refer to your member handbook to find information about the details of your referral process.
Insurance
We are members of several insurance plans. We will bill all insurance carriers for you.
Please bring a copy of your current health insurance identification card to each visit. Remember that your coverage is a contract between you and your insurance company. It is your responsibilty to understand the basics of your coverage. You are responsible for your bills regardless of what your insurance pays.
Co-payments are required at the time of service. If you have difficulty with payments, please discuss special arrangements for extended payments with our billing department. They can be reached at (503) 233-6940 ext. 2303
Laboratory/X-ray
We have a full service high complexity lab in our office and are able to perform the majority of the tests ordered by your provider. Lab appointment hours are 7:40 AM to 5:30 PM. Lab tests are done on site and are filed directly into your medical records. Some laboratory tests may be sent out to a reference lab. If this is the case, you will receive a separate statement from that facility. The majority of our send out labs are performed at OML (Oregon Medical Laboratory), which is also interfaced with our medical records system. You will be notified of results by either phone or mail. We will inform you about any abnormal lab results to arrange appropriate follow-up care. If you have not heard about your results after 3 weeks, please call us and let us know.
X-ray services are provided in excellent facilities located nearby, there is even a facility in our building. Unless ordered as urgent, diagnostic results can take up to a week to be reported. Once your ordering provider has received the results and has had time to review them, you will be notified.
