If you are a new patient, or have new insurance, please arrive 15 minutes prior to your appointment and bring your insurance card, photo ID, any applicable co-pay, and the completed registration and HIPAA forms. This will help us expedite your registration process.
It is also very helpful to have your medical records transferred to our clinic in advance. You can use our
release of information forms for this. If your appointment is for your child to have a well child check, please complete the intake forms
that where mailed to you or you can download from our forms page.


For routine visits, such as physical and well child exams, please call 6-8 weeks in advance so you may have a better selection of appointment times. PFP strives to provide same day access for urgent issues. If you have an urgent issue please try to contact the office as early in the day as possible, as same day appointments are usually filled quickly.
If you are delayed or unable to keep an appointment, please call us as soon as possible. This allows us to adjust our schedules to accommodate you and our other patients. Our providers make every effort to stay on schedule to keep your waiting time to a minimum. Sometimes we must provide urgent/emergency care or deliver a baby during office hours.
We understand the inconvenience this causes and thank you for the consideration you show to others at this time.

Telephone Calls
Our telephone system has been designed to direct all incoming calls to the appropriate department. Please select one of the options given. If you need to speak to a receptionist, press zero at any time. We have a complete message center, staffed with medical staff and schedulers to help assist and direct your requests.
Our office staff is trained to help with many common questions and concerns. If you have an emergency, or require an urgent response, please let us know. Telephone calls will usually be returned the same day, although this may be after regular daytime office hours. Please make sure we have current daytime and evening contact numbers for you.
- When leaving a message, please spell the patient's first and last name, the date-of-birth, and return phone numbers. Please include a brief description of what you need. We will return your call as soon as possible.

The main doors to the building are locked after certain hours, including Saturday. If you have an appointment in the evening or Saturday, press the intercom button located at the front entrance of our building. This will alert staff to let you in.
