Our goal at PFP is to improve your healthcare and provide you access to your health information, in an easy and convenient manner. Patient Portal is a free and secure way for you to connect to us right from your own home or office.
What is patient portal?
Using a secure username and password, you will be able to:
- Send and receive routine messages about your health.
**please continue to contact your pharmacy for regular medication refills.
- Update your personal information, such as phone, address and emergency contacts.
- Check for normal lab results.
- Request appointments for yourself. View your current/past appointments.
- Review your visit summary
- Print out a condensed copy of your medical record.
Once you have been given your username and password,
The first time you login, please read and accept the 'Terms of use' disclaimer for the portal. You will also be directed to verify your account and recreate your own unique password.
- Multiple attempts (5) at a failed log-in will lock the user out.
- NOTE: Please keep your username and password in a safe, secured place at home.
Patient Portal is NOT for emergencies!!

- I can't see my entire chart...
- Any non-urgent communications will be sent to you online. If at anytime after signing up for the portal you decide that you would rather communicate by phone or US mail, let us know and we will de-activate your account.
- While most of our patients have an excellent user experience, from time to time there are questions. Our staff cannot provide technical support, beyond setting up your account, and helping with log on issues. Please be sure to read the 'Help' topics provided and view the 'Commonly asked questions' section before calling our office.
ACCOUNT VERIFICATION
AND PASSWORD RESET
If you are having trouble accessing your portal account, please use the 'forgotten password' link on the portal log on page. Your username is commonly composed of first name, last name (capital letters) and either your year of birth or the last 4 digits of your SS#. No spaces or middle initial.
You will be directed to verify your account and reset your password. (Weak passwords will not be accepted). If you continue to have issues, please call the practice directly.
