What is a Medical Home?
The Medical Home model places the primary care providers at the center of patient care. The primary care provider will work with the RN Case Manager as the bridge to integrated care for each assigned patient. Under this pilot project, a specially trained registered nurse acts as a navigator, partnering with each patient to understand exactly how best to care for them. This nurse ensures your health needs are promptly addressed by actively involving other members of the team, including the patient’s primary care physician, specialists, hospitals, community resources/agencies, and health plans within reasonable timeframe.
