What is a Medical Home?
The Medical Home model places the primary care providers at the center of patient care. The primary care provider will work with the RN Case Manager as the bridge to integrated care for each assigned patient. Under this pilot project, a specially trained registered nurse acts as a navigator,
partnering with each patient to understand exactly how best to care for them.
This nurse ensures your health needs are promptly addressed by actively involving other members of the team, including the patient’s primary care physician, specialists, hospitals, community resources/agencies, and health plans within reasonable timeframe.

"I treat people of all ages. My goal is to keep them healthy and active, and participating in whatever activities they enjoy. This includes acute injuries, chronic injuries, exercise prescriptions for anyone that wants to increase their activity level. I also enjoy treating patients with medical conditions (diabetes, cardiac, etc) who want to be active and perform the activities they love as much or as well as they want."
Dr. White is a family practice doctor whose areas of interest include sports medicine and obstetrics. Utilizing new state-of-the-art equipment, he will perform ultrasound guided procedures along with circumcisions, vasectomies, and specialized gynecology procedures and exams.
In his spare time Thayer enjoys spending time with his family and playing with his daughter. He also enjoys running along the waterfront and hiking in the gorge.

Portland Family Practice is among 13 other medical groups in the Portland area to be selected to participate in the Oregon Health Leadership Coalition Pilot Patient Centered Care Project. The patient-centered care project employs a new model of primary care delivery. This innovative project is a partnership between insurance plans and providers. Specific patients have been chosen by their health plan to participate in this Case Management program.
Portland Family Practice is excited about this unique opportunity to join together with health plans and the opportunity it offers to improve patient care.
